Registered Nurse / Case Manager / Per Diem
We are seeking an LPN or Registered Nurse RN for an immediate opening in Edmonds, WA! This is a local, physician owned, licensed and accredited facility. Their call is to assist individuals remain in their homes by providing a variety of services to meet patient needs.
We offer the following benefits:
- $30.00 to $45.00 /hour, based on experience

Job Responsibilities
The LPN or RN gives professional care in accordance with the physician/provider treatment plan and the nursing process of assessment, diagnosis, planning, implementation and evaluation.
Additional responsibilities for the Case Manager role include:
- Coordinates total patient care by conducting comprehensive health and psychosocial evaluation, monitoring the client's condition, promoting sound preventive practices, coordinating services and educational and training activities
- Evaluates the effectiveness of nursing services to the patient and family
- Prepares and presents patient's record to the Clinical Record Review Committee, as indicated
- Consults with the attending physician concerning alterations of Patient Care Plans, checks with the appropriate supervisor and makes changes, as appropriate
Job Requirements
We are looking for dedicated and professional LPN or RN who are committed to providing exceptional patient care and service. It is also important that you are highly organized and that you demonstrate excellent communication and interpersonal skills.
- Must be an RN and LPN in the State of Washington and graduated from an accredited School of Nursing
